Active Customers (+/-) Last 12 mo.

How It Works
Occurrence
Select IN, OUT or INCIDENT from the Occurrence options.
Identity
Select a Company from Identity or add to the OTHER Field.
Detail
Provide Detail of Any Incidents. This field will only show up if “Incident” is selected in Occurrence.
GID
Select your GID from GID.
